If he had a massive MI and required airlift to OHSU for a ventricular assist device from heart failure, that could be in preparation for a transplant. I hope that isn't the case as the outcomes for that can be pretty grim, but most everything else could have been handled here without the need for transfer. Just speculating and I hope I'm wrong.
